HALO offers Short & Long-Term Disability insurance through BCBS IL Ancillary. If you are enrolled in Medical, HALO covers the cost of your disability insurance.
Short-Term Disability
Short-Term Disability (STD) replaces your weekly income while you are out of work following a non-work-related accident or illness. This coverage will replace 66.67% of your weekly income, up to a weekly maximum of $1,500.
- Benefits go into effect on the 8th day of a qualified disability.
- Maximum of 11 weeks with doctor certification.
- If you are enrolled in the medical plan, short term disability will be provided by the company.
- If you are not enrolled in the medical plan and wish to be covered you must purchase the plan along with long term disability and basic life and disability insurance.

Long-Term Disability
Long-Term Disability (LTD) protects your income by providing you with a percentage of your income while you are disabled. Benefits begin after 90 days of a qualified disability until age 65.
- This coverage will replace 60% of your monthly income, up to a monthly maximum of $10,000.
- If you are not enrolled in the medical plan and wish to be covered you must purchase the plan along with long term disability and basic life and disability insurance.

- Long Term Disability Summary (Class 4): Administrative Employees, not-enrolled in medical, earning under $200k, annually
- Long Term Disability Summary (Class 5): Commissions Employees, not enrolled in medical
- Long Term Disability Summary (Class 6): Administrative Employees, not enrolled in medical, earning over $200k, annually
