HALO offers Basic Life and AD&D insurance through BCBS IL Ancillary. If you are enrolled in Medical, HALO covers the cost of your Basic Life insurance. If you are not enrolled in Medical, you must purchase Basic Life and AD&D coverage. Basic Life and AD&D coverage is two times your annual salary up to a maximum of $250,000. Employee benefits are reduced by 50% at age 70.
You have the option to purchase additional life coverage for yourself, spouse, and dependent children. Additional details can be found below. Follow the Beneficiary Designation Instructions, to setup your beneficiary allocation.
Supplemental Life
Premiums for employee and spouse/partner vary based on your age and will increase according to the age ranges below and your age on January 1 of every calendar year.
- Employee coverage is available in $10,000 increments up to $400,000
- Spouse/partner coverage is available in $5,000 increments up to $50,000
- The guaranteed issue amounts are $200,000 for employee and $30,000 for spouse/partner
- To purchase coverage over these amounts or after initial eligibility, you will be required to submit an evidence of insurability (EOI) form which will be reviewed and approved/rejected by BCBS IL Ancillary.
- Spouse/partner coverage cannot exceed 50% of employee coverage
- Spouse/partner supplemental life coverage terminates at age 70
- Dependent Life Insurance is available as a $10,000 benefit.
- Benefit Amount from birth to 6 months: $250
- Benefit Amount after: $10,000
- Available for dependent children Under age 19 (or under age 26 if they are full-time students), as long as you apply for and are approved for coverage for yourself. Premium includes all eligible children.
