Medical, Dental, and Vision coverage ends on the last day of your termination month. You may be entitled to continue medical, dental, and vision coverage for a period of up to 18 months pursuant to COBRA law. (See COBRA Premium Overview for cost information) WEX Benefits will mail COBRA enrollment materials to your home within two weeks of your termination date.
• Group Term Life and AD&D and Supplemental Term Life, policies will end on the last day of employment. Should you wish to convert or port your life, AD&D policies with NYL, refer to the application below. This must be completed within 31 days of separation. o Application for Portability of Voluntary Term Life Insurance
• Short-Term and Long-Term Disability, Critical Illness, Accident, Hospital Indemnity coverages terminate on the last day of employment.
• Healthcare & Dependent Care FSA plans will allow you to continue to submit expenses that were incurred prior to the last day of employment, for up to 30-days following your separation date. Any unused funds will be forfeited.
• Transit/Commuter FSA will allow you to submit expenses that were incurred prior to the last day of employment, for up to 90-days following your separation date. Any unused funds will be forfeited.
