Post-Employment Information

Below are steps you need to take on or before your last day with us.  We also have provided some helpful information to make your transition easier.  If you have questions, please contact HR.

Benefits

Medical, Dental, and Vision coverage ends on the last day of your termination month. You may be entitled to continue medical, dental, and vision coverage for a period of up to 18 months pursuant to COBRA law. (See COBRA Premium Overview for cost information) WEX Benefits will mail COBRA enrollment materials to your home within two weeks of your termination date.

  • Your coverage will be reinstated with the applicable insurance carrier after your elections and payment have processed with WEX.
  • The coverage becomes effective the 1st of the month, following the date of your benefit termination. Given you enroll within the applicable timeframes, if you experience out-of-pocket expenses while waiting for COBRA coverage reinstatement, you can resubmit any claims retroactive to the effective date of the offer of coverage.
  • Keep any ID Cards for benefits that you will be continuing.
    • Group Term Life and AD&D and Supplemental Term Life, policies will end on the last day of employment. Should you wish to convert or port your life, AD&D policies with NYL, refer to the application below. This must be completed within 31 days of separation.     o Application for Conversion of Group Term Life
    o Application for Conversion of Group AD&D
    o Application for Portability of Voluntary Term Life Insurance
    • Short-Term and Long-Term Disability, Critical Illness, Accident, Hospital Indemnity coverages terminate on the last day of employment.
    • Healthcare & Dependent Care FSA plans will allow you to continue to submit expenses that were incurred prior to the last day of employment, for up to 30-days following your separation date. Any unused funds will be forfeited.
    • Transit/Commuter FSA will allow you to submit expenses that were incurred prior to the last day of employment, for up to 90-days following your separation date. Any unused funds will be forfeited.
