Samaritan Fund Program

Halo partners with the Samaritan Fund Program.  This program aims to provide financial relief to those facing significant medical challenges or the burden of high-cost medications.  Their mission is simple:  To bring financial peace of mind and hope for healing to individuals facing major medical illnesses.  The Samaritan Fund Program sources funds from Samaritan Sponsors to pay for all medical expenses.  Without the financial burden of medical bills, Samaritan Fund Participants can find the Peace of Mind to Heal.

Participation in the Samaritan Fund Program (SFP) is voluntary and entirely at your discretion.  This program is confidential and free to you and family members.  Should you be deemed an ideal candidate, the program will assist in identifying a health plan that both you and your healthcare providers agree upon, then the Samaritan Fund Program will source you with funds from Samaritan Sponsors to pay for medical expenses.  Ultimately, the choice to participate is yours to make.

Getting Started:

To apply, complete the HIPAA Authorization Form.

  • This form enables a representative of the program to contact you directly and assess whether the program is a suitable fit for your needs.
  • If approved, the Samaritan Fund Program will assist you in enrolling in an individual health insurance plan along with sourcing financial assistance that should eliminate the out-of-pocket costs for insurance premiums and medical care.
  • Applying and exploring this offering is straightforward.  However, it is important to note that there is a limited timeframe for making changes to your group plan election.  Changes can only occur during a qualifying event or within the annual enrollment period. The deadline to complete and submit your HIPAA form is no later than October 31, 2024.

How the Program Works

The Samaritan Fund is there to help support you throughout this process.  You can expect the below.

  • Complete and electronically submit a short confidential Medical Information Release form
  • A representative from the Samaritan Fund Program reaches out to explain the program, answer questions, and collect your information and story.
  • If eligible, SFP will explore your insurance options and what amount of funds would satisfy your out-of-pocket cost.
  • If you wish to proceed, SFP will help you submit a customized application to participate in the Samaritan Fund Program.
  • If accepted, SFP will walk you through setting up Samaritan Fund Program, issue a Samaritan Fund Program Debit Card to take care of the expenses associated with your medical care.
  • Once approved, you will need to disenroll from HALO’s Health Insurance during Open Enrollment.
    • If you disenroll from Medical, you will no longer get company-provided Life Insurance and Disability coverage.  You will have the opportunity to enroll in these coverages during Open Enrollment.
    • If you are currently enrolled in a HDHP, you may lose your eligibility to contribute towards an HSA.
    • Eligibility for other benefit programs is not impacted, with a drop in your Medical insurance.
  • The program is evaluated annually for eligibility.
